The basics

What is an ovarian cyst?

An ovarian cyst is a fluid-filled sac on or in an ovary. They are very common in women who still have periods, and most are found by chance on a scan.

The most common type is a functional cyst, which forms during ovulation and usually disappears within one to three cycles. Other types include chocolate cysts from endometriosis, dermoid cysts and cystadenomas, which do not go away on their own.

The type, size and look of your cyst on ultrasound decide what happens next. Most cysts are benign, and your doctor tells you clearly if anything needs further tests.

Know your cyst Common types of ovarian cyst

  • Functional (follicle or corpus luteum) cyst, often resolves by itself
  • Endometrioma or chocolate cyst, linked to endometriosis
  • Dermoid cyst (mature teratoma), contains tissue like hair or fat
  • Cystadenoma, a benign growth from the ovary surface
  • Many small follicles, which can point to PCOS

Your options

Ovarian cyst treatment options

Treatment ranges from doing nothing at all to surgery. Most women need one of the first two.

Watchful waiting

A simple cyst is rechecked by ultrasound after 6 to 12 weeks. Many disappear on their own, and your doctor may suggest checks twice a year.

Simple functional cysts

Medicine

Hormone pills or other medicine may be used to calm cysts linked to endometriosis or to stop new functional cysts forming, plus painkillers when needed.

Recurring or painful cysts

Sclerotherapy or surgery

Persistent cysts can be drained through the vaginal wall and treated with a sclerosing agent, or removed by laparoscopy. See ovarian cyst removal.

If we watch it

What watchful waiting means

Watching is an active plan, not ignoring the cyst. You come back for a scan after a few cycles, usually just after your period, when functional cysts are smallest.

If the cyst grows, changes or causes pain, your doctor talks about the next step.

After the scan

What your ultrasound result means

Here is what usually happens for the most common findings.

Simple cyst, small Thin wall, clear fluid

Usually functional. Recheck after 6 to 12 weeks. No treatment needed if it goes away.

Chocolate cyst Endometrioma

Medicine to control endometriosis, and sclerotherapy or surgery if it is large or affects fertility. See endometriosis treatment.

Dermoid or persistent cyst Will not go away by itself

Removal is usually advised, often by laparoscopy, to prevent twisting or growth.

Unclear features Solid parts or unusual blood flow

More tests, such as a ROMA blood test or MRI, and referral to a hospital if cancer cannot be ruled out.

Risks of treatment, such as infection, bleeding or a cyst coming back, are explained before any procedure.
