The basics

What is endometriosis?

In endometriosis, tissue similar to the lining of the uterus grows on the ovaries, the pelvic lining, the bowel or the bladder. It reacts to your monthly hormones, which causes inflammation, pain and scarring.

When it forms a cyst on the ovary, this is called an endometrioma or chocolate cyst. When similar tissue grows into the uterine muscle, it is called adenomyosis, a close relative that often causes heavy, painful periods.

Endometriosis cannot be fully cured with one treatment, but it can be well controlled. The plan depends on your pain, your scan, your age and whether you want children.

Common signs Symptoms of endometriosis

  • Period pain that gets worse over the years or stops painkillers working
  • Pain during or after sex, deep in the pelvis
  • Pelvic pain between periods
  • Pain with bowel movements or peeing during your period
  • Difficulty getting pregnant

Worth knowing

  • A normal ultrasound does not rule out endometriosis. Small spots on the pelvic lining only show on laparoscopy
  • Painful periods are not always endometriosis. See dysmenorrhea relief
Who should come in

When to see a doctor about endometriosis

On average, women wait years for a diagnosis. You don't have to. Come in if any of these sound familiar.

Situation Why it matters
Painful periods You miss work or school, or painkillers no longer help
Pain with sex Deep pain that you feel inside, not at the entrance
Known cyst A scan abroad showed an ovarian cyst or chocolate cyst
Trying to conceive No pregnancy after 6 to 12 months, depending on your age
After surgery You had endometriosis surgery before and pain has come back

Bring any earlier scan, MRI or surgery report. It helps your doctor compare.

Get seen the same day if you have:
  • Sudden, severe one-sided pelvic pain
  • Pelvic pain with fever or vomiting
  • Fainting or heavy bleeding

These can mean a burst or twisted cyst. Message us on WhatsApp straight away, or go to the nearest hospital emergency room.

Your options

How endometriosis is treated

Most women start with medicine. Procedures are added when cysts are large, pain stays or fertility is a concern.

Hormones and pain relief

Anti-inflammatory painkillers plus hormone treatment, such as progestin pills or a hormonal IUD, calm the tissue and often reduce pain a lot. Many women use this for years.

First step for pain

Cyst sclerotherapy

For chocolate cysts, a needle through the vaginal wall drains the cyst and treats its lining under ultrasound. Your ovary stays in place and there is no scar on your belly. See ovarian cyst removal.

Endometriomas

Laparoscopic surgery

Through small cuts, the surgeon confirms the diagnosis and removes endometriosis spots, cysts and adhesions. It is done in the B2 surgery center.

Living with it

A long-term plan

Endometriosis often comes back when treatment stops, so most women need a plan until menopause or pregnancy. Regular ultrasound every 6 to 12 months checks any cysts.

If you want to get pregnant, tell your doctor early. Protecting your ovaries changes which treatment is best, and an AMH test can show your ovarian reserve.

What each treatment can and cannot do

Every option has benefits and downsides. Your doctor talks them through so you can choose with confidence.

Hormone therapy Pills, IUD or injections

Often reduces pain well. Side effects can include spotting, mood changes or bloating. It does not remove cysts and you cannot get pregnant while on most options.

Sclerotherapy For endometriomas

Keeps ovarian tissue and avoids surgery. Cysts can come back, so follow-up medicine and scans are advised. Rare risks are infection or bleeding.

Laparoscopy Removes visible disease

Confirms the diagnosis and can ease pain. Risks include bleeding, infection and organ injury, and cyst surgery can lower ovarian reserve.

Adenomyosis options If the uterus muscle is involved

HIFU can treat adenomyosis without an incision. See fibroid and adenomyosis treatment.
